Update the Windows section to more clearly note that you need 2010 version of Visual C++ and also describe how to do the build in this newer version (I don't even get a build menu in 2010 version). Also, discuss the option of checking out the source from svn rather than downloading a tarball

Switch to -sn and -Pn as the new preferred synonyms for -sP and -P0.
...
This establishes a more regular syntax for some options that disable
phases of a scan:
-n no reverse DNS
-Pn no host discovery
-sn no port scan
Also, the -sP was possibly misleading because the 'P' suggests "ping
scan," when you can now do more than just pinging when you disable port
scanning. For example, -sC -sn and -sn -Pn --traceroute make sense.

Expand the sorting text to include all the letters of the index entry text when

using sortas in an index entry. Previously I had things like
<indexterm><primary sortas="nmap"><filename>.nmap</filename> filename extension</primary></indexterm>
<indexterm><primary sortas="nmap"><filename>.nmap</filename> directory</primary></indexterm>
which, because they have the same sort key, would result in one index entires
under either ".nmap filename extension" or ".nmap directory", with the page
numbers for both. Now I just spell out the full text: sortas="nmap filename
extension".
